Vaccinated patients who are two weeks past their second dose may be seen in person for individual sessions upon proof of vaccination.



OUR OFFICE IS ONLY OPEN TO STAFF AND THOSE WHO HAVE MADE PRIOR ARRANGEMENTS FOR IN-OFFICE APPOINTMENTS.
WALK-INS ARE NOT PERMITTED.

CURRENT PATIENTS

All group sessions are being conducted using Zoom's HIPAA-compliant telehealth system. Vaccinated persons may opt to be seen in person for their monthly individual sessions. Please contact the front desk for more details.


PAYMENTS 

Payments can be made over the phone with a debit or credit card during limited business hours, or online at any time. If you must pay with cash, please contact the front desk to scheduled a time to come into the office to do so.

INCOMING PATIENTS 

All intakes will be done over the phone. Due to current limitations on obtaining documents from other agencies, there may be difficulties scheduling intakes for those who were assessed elsewhere. Please call for more information on how to obtain docs and getting your appointment scheduled.


DRUG/ALCOHOL ASSESSMENTS

Assessments will be done via Zoom. You will receive a link for your appointment after the following steps have been taken:

1. Create your Patient Portal.

2. Fill out the three-page Addiction Treatment Application in its entirety.

3. Take clear pictures of your court documents, ID, and insurance card (front AND back) and email them to support@affinitycounseling.net. If you are being assessed due to a DUI or other driving infraction in WA, you must also send your FULL Washington State driving record, which can be obtained at dol.wa.gov.

4. Pay the $90 assessment fee 48 hours prior to your appointment. Payments can be made over the phone or online at https://affinitycounseling.maxcheckout.com/.

5. Check your email the morning of your appointment for your Zoom invite. Check your junk or spam folder if you don't see it in your inbox.

6. Be on time for your appointment. The clinician will disconnect after 10 minutes.

You will not receive an appointment invite if payment and/or docs have not been received.
